This form is used to apply for approval of a proposed headstone/monument for a Cemetery Plot or Plots, at the Town of Frisco, Colorado Cemetery.

Headstone Dimensions (Required for New Installments)
Do not only provide numbers here. Must provide designations for dimensions, such as: Length, Width, Height, Thickness, and Material for each foundation and actual headstone piece. Use only feet/inches for measurement unit. The area of the memorial face shall not exceed 10 square feet. The base of the memorial shall not exceed 4 feet in width or 2 feet in depth. Any marker shall be at least 4 inches thick. The foundation for such base shall be determined by the size of the monument or marker. In no case shall a marker be more than 42″ in height including the base from grade.

Installation/Engraving Service Rules Acknowledgment*
- I have read and understand the Installation/Engraving Service Rules listed below.
Installation/Engraving Service Rules: 1. No driving anywhere other than the paved road through the Cemetery, 2. Must provide 72 hours minimum notice prior to installation, 3. All spoils will be removed from the site by the service professional at the conclusion of service performed, 4. Concrete foundations shall not be exposed or above grade, 5. No stakes allowed, 6. All tools and materials are furnished by the contractor and not by the Town, 6. The marker will only be installed within the pre-marked area by the Town, including following the arrow indicating direction the headstone must face, 7. Installation work can only be performed between Mondays-Thursdays 7:00AM-3:00PM, and Fridays 7:00AM-Noon, and 8. The marker must be placed at the head of the Plot facing the foot of the Plot, there are no exceptions. I understand that all Frisco Cemetery Rules and Regulations must be followed at all times and under all circumstances, and that I can email the Town Clerk at cemetery@townoffrisco.com with questions.
