Tobacco Retailer Surrender of License Notification

This form is used to officially notify the Town of Frisco that a Town of Frisco Tobacco Retailer Licensee is surrendering their Town of Frisco Tobacco Retailer License. This is an irreversible action, and supplemental documentation and verification will be requested in addition to this form by the Town of Frisco to verify the surrendering of the license. Town of Frisco Tobacco Retailer Licenses cannot be transferred.

I am notifying the Town of Frisco, Colorado that I hereby surrender my Town of Frisco, Colorado-issued Tobacco Retailer License. I understand that §110-94(C) of Frisco Municipal Code, a Tobacco Retailer License cannot be transferred, but must be surrendered and then a new application for the same premise must be submitted at a later date.*
